It's best if you use a computer to fill out our application. This is a PDF document so some version of a PDF viewer (like Adobe) is required. After selecting “click here to download”, the document will open in a separate tab. Click the download icon in the top right hand corner of your screen. You will then be prompted to save the document to your computer. When completed, open the document (you will need an application like Adobe to view and fill out this document) and fill in the editable blanks.

Take your time, attention to detail is important. If you have any questions ask a friend. Try not to leave anything blank. Fill out the document AFTER downloading to your computer. When locating your downloaded application, it may help to right click on the document, go to “open with” then select “Adobe Acrobat Reader” and this will give you the tools you need to complete and sign the application. Please sign the document with the programs pen tool or add an e-signature. Or you may print the filled out document, sign it, then scan it and email it to us. This application must be submitted via email to apply@weekleysice.com.
